Important in making our model work is a focus on personal outcomes and the development of a support plan. Through the planning process individuals identify what their aspirations, goals and needs are, as well as how they are going to be met.
We also ensure that the process is a reflective one, with our outcome-focussed reviews, facilitating discussion with the person and the people who are close to them, so that they have the opportunity to make informed choices about current and future care and support needs based on shared decision making.
The result is that participation becomes an ongoing and flexible process and not an end in itself as we know that support plans change over time as people’s needs, wishes and personal budgets change.
